That member may be a disabled child, or an injured adult, or perhaps someone with Alzheimer&#8217;s disease. More families are opting to keep their loved ones at home and to care for them there instead of institutionalizing them. Caring for someone in need at home is time consuming and it takes a lot of energy. The person who does most of the care is called the primary caregiver. It is this person who will need to invest in respite care.</p><p>Respite care provides assistance to those who care for the elderly family members, or community members who are in need of care in their homes. Respite care gives primary caregivers a break when they need it, and during that break ensures that the loved one is receiving the attention and care that they need.</p><p>There are different types of respite care such as the traditional in-home care, adult day care, skilled nursing home care, home health care, and short-term institutional care. Respite care can take the form of informal or volunteer respite care which is when other family members, friends, neighbors, or church volunteers offer to care for the loved one while the primary caregiver attends to personal business, runs and errand or just has some time off from the duties of caring for others. Many communities have programs where volunteers are matched with caregivers to ensure that they get the relief they need, when they need it.</p><p>Adult day care consists of programs designed to provide care and companionship for those who are disabled or frail and need assistance or supervision during the day while the primary caregiver works at a job, or attends to personal matters. Primary caregivers can do whatever they need to do and be assured that their loved ones are being well cared for by the adult day care center.</p><p>In-home respite care is care that involves four different types of services for the disabled or elderly that include companionship, when someone needs to be supervised, entertained or just visited while the primary caregiver is away. It can also mean homemaker services such as light housekeeping, or the preparation of meals, or even shopping. It can also mean personal care services such as helping the loved one bathe themselves, or helping them to get dressed, or go to the bathroom. Every person&#8217;s needs vary when it comes to identifying their own breaking point, but by and large there are several times when respite care is an identified need: usually it is the care of an elderly individual suffering from dementia, being the primary caregiver of a senior citizen who otherwise might need to enter a nursing home, tending to a disabled child, living with a loved one who is under hospice care, and also when you are adopting or fostering a child.</p><p>Various forms of respite care exist. By far the most common form is the one that takes place in your home. A respite care worker will be sent via an agency to your home where she or he will take over your place while you take some time off. You do not need to leave the premises although many kinship caregivers opt to do so. In many cases the respite caregiver is well trained in basic medical procedures and also has been trained extensively to provide a simple emergency care protocol as required. In your absence, he or she may provide a meal for the family member, help with basic toileting and other personal hygiene tasks, and also provide some form of companionship. In some cases, respite caregivers also offer services with respect to giving medicines, although this needs to be discussed ahead of time as not all agencies will offer this particular service.</p><p>Respite care in the US is greatly regulated and the rules surrounding it determine how often a kinship caregiver may take advantage of this kind of care. Depending on who foots the bill for the service, the incidents of respite care may be limited to a certain calendar day allowance per year. In addition, the age of the patient greatly influences the kind of available care appointments the caregiver may enjoy. Some kinship caregivers are initially unwilling to make use of this available care option and some wonder why they actually should consider it at all.</p><p>The fact of the matter is quite simple: no matter how dedicated you are to the patient, the fact that caring for an individual on a 24-7 basis is tiring and wearing will eventually catch up with you. To counteract fatigue leading to potential neglect and abuse, the idea behind respite care specifies that the kinship caregiver gets temporary time off and gets to just leave and do something they might have put off. In some cases the caregiver may opt to reconnect with a spouse on a weekend getaway while in other cases she may choose to just take the time to run some much needed errands that thus far had been going by the wayside. Sometimes this institutionalization took place as early as childhood when the parents found they could no longer adequately care for the child, while at other times it took place in adulthood when the parent caregiver found that because of personal failing health they could no longer provider the services required to keep the patient healthy and functioning at the highest level, or sometimes the institutionalization took place at the death of the caregiver.</p><p>In cases where the caregiver is living but is reaching a point where the consistent care required is becoming overwhelming in scope and time commitment, respite care reduces incidents of institutionalization. Whether the individual is suffering from a developmental disability that makes independent functioning virtually impossible, or &#8211; in the case of a child &#8211; the primary family ties are severed and the child enters the foster care system where finding a family willing and able to commit to the sudden needs for constant care is usually hard to begin with, respite care makes it possible for caregivers to perform the care needed for the individual while at the same time also taking into consideration that the foster family of the kinship caregiver has personal needs and familial obligations to attend to.</p><p>There is another facet that is frequently forgotten in the discussion over respite care: reduction of institutionalization is achieved by offering parents or foster families a workable alternative to traditional babysitting or peer care programs that by and large are unacceptable for those suffering from moderate to severe mental or physical handicaps. Although many such individuals probably have offered at one point or another to help out with the individual, the fact that they are usually unskilled, lack the acumen required for caring with medical emergencies, and overall will most likely not possess the necessary stamina and strength that is required to deal with a child who is growing in strength and may not be cooperative, all contribute to the hesitation of the parent to avail themselves of such assistance.</p><p>In other cases it is the somewhat misplaced feeling of guilt that has the parent believing that enlisting the help of a caregiver is much the same as abdicating responsibility. When the demands on the individual become so overwhelming that the parent is not sure how to keep healthy and focused themselves, the odds are good that an overtired, overwrought individual may opt for an institutional setting where their grown child may receive the best care possible. Fortunately the advent of respite care offers the best of both worlds in that trained professionals now come to the home and take care of the child on a temporary basis while the parent may take a bit of a rest. Kinship caregivers love the patient for whom they care; perhaps it is a child, a parent, a close relative, or even a relative by marriage who has come to mean a lot more than just an extended family member. When this relationship which initially started out entirely differently suddenly shifts in power and responsibility to such an extent that the patient&#8217;s health, overall wellbeing and even life is depending solely on the caregiver, frustration is certain to set in sooner rather than later.</p><p>Aggravating this penned up frustration for US caregivers is the sheer volume of red tape that seems to surround each and every interaction the patient may be facing with a physician, a governmental agency, and sometimes even with commercial institutions that have little compassion for the ailing individual. In addition, considering that peers who may have never been in the position of kinship care giving before do not hesitate adding pressure to the caregiver for missed appointments and a seeming unwillingness to commit to schedule changes, and the landscape is ripe for a blowup.</p><p>Finally, good advice from outsiders is never far away, and some of the suggestions given are so outlandish and insensitive that it is a wonder kinship caregivers are not more frequently in the news for blowing up. Sadly, when these frustrated US caregivers do make the news, it is usually while being arrested for abuse or neglect of the very patient with whose care they were entrusted. Although it is simple to appear horrified at the tales that are told on the evening news, there is precious little that could have been done about it &#8211; until now. The goal of this legislation is the empowerment of group and organization based respite programs with the help of a fiscal infusion from the very onset.</p><p>As funding permits organizations to train volunteers and also hire professional respite care providers, the level of care that may be offered to families in need soon is paraprofessional. At this time the organization is added to a database identifying those groups which have trained care providers that are available for a variety of in home respite services. To ensure that the organization starting up the program can continue on with the services, the state of Oregon has decided to continue funding groups that are members of its database and thus the initial investment is not lost but instead supported many times over.</p><p>Families in need of help with respite care may dial up the database at any time via the Internet and find out what the costs for the care are, if there are referrals available for the care of high maintenance and high need patients, and also to allow families from adjacent communities to find care in perhaps the next county or city.</p><p>The creation of the database has proven a most valuable tool in Oregon in that it offers an immediate method of connecting willing respite workers, the organizations that represent them, and families in need of their services. In addition, those interested in becoming respite care providers also find a unique way of networking and ascertaining how to train and specialize for work as a respite caregiver.</p><p>Another valuable function the database has shown to provide is a pinpointing service of areas &#8211; either geographically or medically &#8211; that are considered underserved. Such areas may be targeted with the help of outreach programs in order to establish organizations able to take on a respite care program and then work with them to recruit the workers.</p><p>There has been some confusion with respect to whether or not respite care services are provided free of charge to Oregonians. At his point in time this is handled on a case by case basis, depending on the family&#8217;s ability to pay, but by and large these services are for pay. Some of the exceptions are the respite care appointments offered by charitable organizations and faith based communities.</p><p>While these groups will offer their services free of charge, finding one that does not currently have a long waiting list is a harder task by far. Generally speaking, these coveted appointments are usually snapped up as quickly as they become available and families may need to either wait for a considerable length of time or opt for the for pay route. Although there are always friends and family members who are at times acting in the respite care volunteer capacity, it is the adequately trained individual that is sought out by those who are lacking the appropriate social network &#8211; and these individuals are becoming the majority by a wide margin.</p><p>If you do not believe how disastrous this potential shortage of well trained respite care volunteers is going to be to the United States healthcare system as you know it today, consider the short term and also long term possibilities.</p><p>* Family crisis situations are not dealt with on a timely manner. While a half day respite care provision may have enabled the breadwinner of the family to find a job due to impending layoffs, the failure of having such care available leads to the actual realization of the job loss and subsequent homelessness of not only the family but also the patient.</p><p>* Child abuse runs rampant in families where the primary parent caregiver does not receive some time off. Remember that dealing with a possibly terminally ill or gravely disabled child takes a toll not only physically but also emotionally and psychologically. Without time to attend support group meetings or just get away, the caregiver will eventually lose control of the penned up frustration, anger, despair, hopelessness, and rage and take it out on the child itself.</p><p>* Elder abuse is underreported in many instances but it is a very real outcropping of lacking resources for respite care. Adult caregivers are essentially providing support and care for two separate families and households and pulling double time without any breaks or days off is going to wear rather thin. Remember that elder abuse is not always achieved via actual physical abuse but also through neglect when the caregiver simply curtails the caring times and activities. This of course directly impacts the overall health of the elderly patient.</p><p>The result for any and all of these actions is an increase in hospitalizations, long term care arrangements, professional home health needs, and also the institutionalization of individuals who would ordinarily thrive in their own homes. As such, the insurance industry, the welfare system, and private donors are going to find themselves taxed to the limit. Before long, respite organizations are going to turn away clients and the cycle only worsens.</p><p>A collapse of the US healthcare system&#8217;s stability is imminent &#8211; unless the shortage of respite care volunteers is reversed. Although it is not likely that this will happen in a large scale, the creation of incentive driven volunteer programs may be a good beginning for encouraging more individuals to become trained and then make use of their training for charitable purposes. You may opt for an adult daycare setting or for an in home caregiver to visit the patient in your absence. This may be either a volunteer respite worker or someone who is from a professional respite organization.</p><p>* Contact a home healthcare agency and see what services might be covered. When combined with respite care, the visits of a certified nursing assistant can give you a bit of an extra breather when it comes to meal preparation and also personal hygiene for your loved one.</p><p>* Have a medical emergency bracelet engraved that identifies not only you, the loved one, but also the treating primary care physician. Do not betray confidences and do not allow yourself to gossip either about your friend or the loved one in her care. Sure, it would be easy to commiserate and vent your frustration, but in so doing you are gossiping and there is little doubt that your little verbal indiscretions are going to come back to haunt you sooner or later. Before considering any scheduling issue, caregivers must come to realize that they themselves matter as much as the person for whom they are providing the care. If the caregiver loses the ability to smile, provide physical help, be emotionally strong, and overall assist the loved one with day to day life, then the quality of care will not only decline but there is a danger that anger and resentment may actually take over.</p><p>Avoiding this problem is imperative but it can only happen when the caregiver acknowledges that the world does not revolve around the loved one. Yes, she or he is important and loved, but not the center of the universe. Frequently the number of specialists, physicians, clinics, and rehab providers involved in the care were intimidating to the foster parent not simply because of the at home monitoring of the child that was required, but also because the time commitment that it would take to keep all the scheduled appointments appeared to be beyond the scope of one individual.</p><p>Since many willing foster parents either foster more than one child or have children of their own, engaging in the care of another child with a host of special needs is not always a viable option, in spite of their overall willingness to step up to the plate and make it happen. To this end many a program has been devised to offer fiscal incentives to prospective foster parents, but overall the results were dismal. Although more potential foster families could be recruited, the number of those looking for the fiscal remuneration as opposed to taking seriously their responsibility to the foster child also went up.</p><p>It was not until respite care was introduced that foster families had the confidence to come forward and apply for fostering special needs children with a large number of medical and also psychological problems. In the United Kingdom and also the United States, respite care for foster families enabled the adults in the family to take some time off from caring for the affected child and simply have a night out, a weekend away, or even take a small vacation. All the while the child was in the capable hands of a trained, professional caregiver who would take over on a short term basis for the foster family.</p><p>Thus refreshed, the foster parents would return later on to take over their duties once again.
